Adult Services aims to help people continue to live in their own homes as independently as possible, whatever their disability or age.

We can offer information and advice to everyone about the range of social care services available and can provide free assessments of the care and support that people need.

Assessment - what help do you need?

Everyone is entitled to an assessment of their needs to see if Hampshire County Council Adult Services Department can help. As a department our main emphasis is on providing help that will keep people living independently for as long as possible. Whether you have a long term disability, have just come out of hospital or are struggling as a result of age or illness the department may be able to help.  

There are several options available depending on the nature of your problems

Are you finding difficulty coping with daily activities?

If you, or of the person you are caring for, are finding difficulty coping with daily activities such as personal care or meal preparation then contact Adult Services. Trained advisors will discuss your situation and determine how urgent your situation is. If it is identified that you require further support you will be referred onto the appropriate care management team.

Do you need equipment or adaptations for your home?

If you are finding difficulty managing in your current housing and feel that some equipment or adaptations will assist you may want to speak to an Occupational Therapist at OT Direct

Do you have a hearing, sight or dual loss problem?

If you have a hearing loss. sight loss or dual loss problem please contact The Sensory Services team where a specialist Advisor will be available to help.

Registering as disabled

Registering as disabled may entitle you to some concessions and benefits, for example, reduced travel costs on buses and trains.

Transition

Transition is when young people aged 14 – 25 with disabilities and complex needs, move from Children’s Services to Adult Services
